• [2026-09-07 Mon 19:32] Refinement to the default title: style the project root name as Org verbatim text. The default #+TITLE is: RAIL for <project name>, where <project name> uses Org verbatim markup (equals signs) around the root directory name.
• [2026-09-07 Mon 19:31] Default title: when the skill provides defaults, the default Org #+TITLE for a RAIL.org should be "RAIL for <project name>", where <project name> is derived from the project root directory name (file-name-nondirectory of rail-project-root). A file that declares its own #+TITLE overrides this default.
• [2026-09-07 Mon 19:27] Precedence decided: the skill provides the default TODO sequence and TAG groups. A RAIL.org file that declares its own #+TODO or #+TAGS overrides the skill default for that axis. The file owns eventual overrides of the skill defaults.

:END: The RAIL skill should own the default set of TODO sequences and TAG groups, rather than requiring every RAIL.org file to declare them in its preamble. Today the tools carry no vocabulary and the file owns both (#+TODO and #+TAGS). This request adds a default the skill provides, so a file that omits #+TODO or #+TAGS still gets a sensible sequence and tag groups. Decide precedence: a file that declares its own #+TODO/#+TAGS should still override the skill default. Update rail-inspect, capture/setup docs, and the SKILL.md setup section accordingly. This is a follow-up to the closing-note redesign and should be done after it.

:END: Replace the body-line closing evidence (- result ::, - blocked ::, - cancelled ::) with Org closing notes stored in the LOGBOOK drawer next to the CLOSED/state-change timestamp.

• Switch RAIL.org preamble to org-log-done 'note, and enable note logging on the BLOCKED and CANCELLED transitions.
• Drive the closing note non-interactively from the tools (org-add-log-setup + org-store-log-note); no minibuffer prompt.
• Scope: DONE, CANCELLED, BLOCKED each write a closing note. The closing note is the natural conclusion of the item and completely replaces the old body lines.
• rail-complete composes commit/tests/model/notes into the note; rail-block and rail-cancel write their reason as the note.
• Migrate the existing RAIL.org, converting the two closed items to closing notes.
• Update SKILL.md, references/tools.md, and the rail-tests.el suite.
