docs Fix test path and stale tool reference text
The test-runner path in SKILL.md assumed a project-local install, while the README installs the skill by global symlink. The tool reference named four fixed tag axes and claimed the project root needs no configuration. The code derives axes from the file, and a shared install must set rail-project-root.
